Types of Corneal Surgery

DSAEK

During Descemet’s stripping automated endothelial keratoplasty (DSAEK), Dr. Nam replaces only the damaged endothelial (inner) layer of your cornea. This is achieved by removing the diseased Descemet’s membrane and endothelium and inserting a thin donor tissue layer. The donor tissue is then positioned carefully to restore your cornea’s function. DSAEK is minimally invasive compared to full-thickness corneal transplants, offering quicker recovery times and improved outcomes for patients with endothelial cell loss, such as those with Fuchs’ dystrophy.

DMEK

Descemet’s membrane endothelial keratoplasty (DMEK) is a procedure where Dr. Nam replaces the damaged Descemet’s membrane and endothelium with an extremely thin layer of donor tissue without disturbing other corneal layers. Dr. Nam positions the ultra-thin tissue in your eye, allowing it to adhere naturally with the help of an air bubble. DMEK provides exceptionally clear vision and low rejection rates, making it an ideal option for patients with endothelial dysfunctions, such as Fuchs’ dystrophy or post-cataract surgery complications.

DALK

Deep anterior lamellar keratoplasty (DALK) is a partial-thickness corneal transplant where Dr. Nam replaces the diseased stroma, the thick middle layer of your cornea while preserving your healthy endothelium. During DALK, Dr. Nam removes only the damaged stroma and replaces it with donor stroma, maintaining the inner layers of your cornea. This approach preserves more of your eye’s natural anatomy, reducing the risk of rejection. DALK is typically used for conditions like keratoconus or corneal scars that do not affect the endothelium.

PK

Penetrating keratoplasty (PK), or full-thickness corneal transplant, is a procedure where Dr. Nam removes all layers of your cornea and replaces it with a healthy donor cornea. This is done when the damage extends through the entire corneal thickness. Dr. Nam will carefully secure the donor cornea with tiny sutures, ensuring it integrates with your eye over time. PK is often recommended for patients with severe corneal scarring, advanced keratoconus, or other full-thickness corneal diseases where partial transplants are not sufficient.

Pterygium Surgery

Pterygium surgery addresses abnormal tissue growth on the cornea, commonly caused by UV exposure or chronic irritation. In this procedure, Dr. Nam removes the pterygium, or growth, and, in many cases, places a conjunctival graft—healthy tissue from another area of the eye—to prevent recurrence. This graft reduces the likelihood of the pterygium returning and improves cosmetic results. Pterygium surgery is particularly beneficial for patients with discomfort, vision impairment, or noticeable redness from the growth.

Superficial Keratectomy

Superficial keratectomy is when Dr. Nam carefully removes irregular or damaged cells from the corneal surface. Dr. Nam smooths the corneal surface, eliminating lesions, scars, or dystrophic tissue that can interfere with your vision or cause discomfort. Superficial keratectomy is ideal for patients with conditions affecting the outermost layer of the cornea, such as anterior basement membrane dystrophy, recurrent corneal erosions, or surface scarring from past injuries or infections. This procedure promotes clearer vision and enhances corneal smoothness.
